Clive Form Builder


What is Clive?

Clive is a Hannon Hill product that enables users to create custom forms, no programming skills required. The forms you make on your web page allow you to store data your visitors fill when they submit a form. Data compiled from your custom forms are easy to access and organize. Clive makes managing forms and submission notifications simple to use and easy to access.

Logging into Clive

Please visit the Clive Access Form page to request access for your department.

For Cascade and Clive Users:

Visit https://app.clive.cloud/login and click on Cascade CMS next to "Other sign in options". Input the full Cascade CMS URL without the quotes: "https://tamuk.cascadecms.com/", then click sign in. You will be redirected to the Cascade login window. Sign in using your KU# credentials.  After successfully logging in you will be redirected back to Clive.

For Clive Only Users:

Visit https://app.clive.cloud/login and sign in using the login credentials assigned to you by the MARCOMM Web Department.

General Settings

To get started with creating forms in Clive, visit the Forms area and click Create a Form.

  • Form Name - Enter a name for your form. This name will be used to identify the form within Clive.
  • Description - Enter a description of your form. This title will be displayed in the final rendered form as a heading above the form fields.
  • Label Positioning - Choose whether you'd like the position of your form field labels to appear at the Top or to the Left of each field.
  • Show Indicators For - Choose whether you'd prefer Optional fields (doesn't have to be filled out before submitting) to be indicated on the form or Required fields (must be filled out before submitting).

Adding Form Fields

Add fields to your form with the Add Field button. Choose a field Type and enter a Label (Title) for the field.

  • Placeholder - Use this to provide an example value or label alternative.
  • Help Text - Use this to provide instructions to the submitter.
  • Required - Makes the field required in order to submit the form.
  • Advanced Settings - Open settings to show the field conditionally based on a value in another field. 

You can also create Custom Fields that allow you to save a form field and its values to share and reuse in your Clive forms. Visit the Forms > Custom Fields area and click Create a Custom Form Field.

  • Convert to a new Custom Field - Save the current field along with its settings as a new Custom Field that can be shared and reused in other Clive forms.
  • Edit Custom Field - Opens a new tab/browser window to edit the current Custom Field.

For radios, checkboxes, dropdowns, and multiple selection fields:

  • Default Value/Option Values - Out of the multiple Option Values you create, you are able to assign one of them to a Default Value that will be displayed as a placeholder.

When finished, click Update to save your field changes. To save your entire Form click Save at the bottom of the page.

NOTE: Clicking Update does NOT save your Form, it only saves your Form Fields.

Reordering Form Fields

If you need to reposition a form field, you can drag and drop it into place with the drag handle to the left of the field:

Field Types

Text Field - Creates a short, one-line text box for short answers.

Long Text - Creates a large text box.

Radios - For use with multiple options, uses a radio button to indicate preference.

Checkboxes - Another type for use with multiple options where selections are made with a checkbox.

Dropdown - Like the Field Type Selector itself, this type creates a drop-down list of the Added Values.

Multiple Selection - For use with Multiple options where multiple selections may be made.

Submission Settings

  • Include Captcha - Prevents spam form submissions by enabling verification to submit your form. (ALWAYS Include Captcha)
  • Submit Button Text - The default submit button text is "Submit", but you can provide your own text.
  • Form Submit Action - Choose whether to display a completion Message or Redirect to a URL (must be fully-qualified, ex. https://www.example.com) when visitors submit the form.
  • PLEASE NOTE: Clive does NOT autosave your work, you must SAVE your work often.

Exporting Form Submission Data to Excel

To export your submission data to excel, click the form you would like to export. Click the Submissions section, then click the "Export as CSV" button on the far right.

Form Notifications

Once your form is created, you can notify subscribers of form submissions by visiting the Notifications area when previewing the form.

Click Notify people when this form is filled out and select subscribers in your site or provide the email address(es) of external subscribers to be notified by email when a visitor completes the form. By default, subscribers will be notified of all form submissions (Always).

Conditionals

You can edit a user's subscription to add conditions for when they should be notified based on values submitted by the visitor. For example, notify the appropriate department contact based on the visitor's selected area of interest.

To do this action visit the Notifications area and click Notify people when this form is filled out. Then click the appropriate persons to receive the notification. (You can also type in an email not listed to receive notifications under Subscribe by email address)

Then, click the pencil icon on the right. Click notify this user.

Click the dropdown box and select Only when Visitor.

From here you can choose specific fields and values to correspond with your notification preferences. When you click your field of choice, you must fill in the whose value section and choose among Is Filled in, Is Empty, Has an Exact Value, or Contains. Click the Save button to save your conditional notification.
