Final Graduation Requirements

Check with your Graduate Coordinator regarding any Departmental deadlines.

Graduate Studies will need the following:

  1. Submit the GS-105 Masters Degree Comprehensive Examination Report (Courses Only)  (DocuSign digital signature routing)

    This link will open a new window, where the student will enter all relevant names and then start the document on its routing. The student fills in their assigned information and the Graduate Coordinator is most often the person who will fill in the rest of the form. Contact our office if your College or Department operates in a different manner.

Graduate Studies will need the following:

  1. GS-103 Research Project Final Submission and Comprehensive Exam Report (DocuSign digital signature routing)

    This link will open a new window, where the student will enter all relevant names and then start the document on its routing. The project supervisor will fill in the rest, and it will route to all who need to sign.

  2. GS-110 Turnitin Report (DocuSign digital signature routing)

    This is required for a thesis, research project, and doctoral candidates. It allows for the remote submission of the Turnitin Report, along with the faculty mentor's signature and any required explanation for similarity rates equal to or greater than 15%. There is an upload button for the faculty member to use to insert the Turnitin Report PDF. 

 *Prior to obtaining any signatures on any forms or pages Project Supervisors are encouraged to send the Student to the College of Graduate Studies to make sure it is in the correct format. Pages with incorrect formatting, official names and/or titles WILL be rejected and a corrected page with new signatures will be required.

Graduate Studies will need the following: 

Step 1: After a successful defense, the student will submit the following final requirements:
  1. Submit the final manuscript ONLINE as a Word file. No hardcopy is needed for the manuscript.

    If you followed a particular journal’s format, you must provide a printed sample and the format guidelines.
  2. GS-101 Thesis Comprehensive Examination Report Form (DocuSign digital signature routing)

    This is required for candidates following the thesis master's degree path. It includes the Comprehensive Exam results as well as documents the Thesis Defense. This link will open a new window, where the student will enter all relevant names and then start the document on its routing. The committee chair is usually the one to enter the exam and defense Pass or Fail information

  3. GS-106 Copyright form (DocuSign digital signature routing)

    This link will open in a new window. The student will fill in the sections assigned to them, and the Chair or Committee member will sign off on it. This form will automatically route to the Jernigan Library Tech Services division when complete, to officially log the copyright information.

  4. GS-110 Turnitin Report (DocuSign digital signature routing)

    This is required for a thesis, research project, and doctoral candidates. It allows for the remote submission of the Turnitin Report, along with the faculty mentor's signature and any required explanation for similarity rates equal to or greater than 15%. There is an upload button for the faculty member to use to insert the Turnitin Report PDF.

Step 2: The completed signature page will be inserted into the final document, which is then emailed to the student with their instructions for how to upload into ProQuest.  It is recommended that the file be uploaded as a Word document, to avoid errors that crop up in the pdf creation process.  
Step 3:

Submit the final manuscript to ProQuest. Follow the instructions to submit the full document.

This file is reviewed one final time.  If all looks appropriate, it will be sent through to ProQuest for publication  and holds on diplomas and official transcripts can be lifted by the appropriate party.

      This is required for doctoral candidates. It documents the Dissertation Defense. This link will open a new window, where the student will enter all relevant names and then start the document on its routing. The committee chair will enter the Pass or Fail information.

    3. GS-205 Qualifying Examination Report Form (DocuSign digital signature routing)

      This is required for doctoral candidates. It documents written and oral examinations. This link will open a new window, where the student will enter all relevant names and then start the document on its routing.

    4. GS-106 Copyright form (Docusign digital signature routing)

      This link will open in a new window. The student will fill in the sections assigned to them, and the Chair or Committee member will sign off on it. This form will automatically route to the Jernigan Library Tech Services division when complete, to officially log the copyright information.

    6. Survey of Earned Doctorates (SED) certificate (optional)

      This is optional to complete for Ph.D. students.
